AMM Healthcare cares about your privacy. This policy explains what information we collect when you use ammhealthcare.com, how we use it, how we protect it, and the choices you have. We follow federal law (including HIPAA) and North Carolina law (including the North Carolina Identity Theft Protection Act), and we give residents of other states the rights their state laws require.
Does this cover my medical records?
No. Your medical records and any health information we collect when you receive care from us are covered by a separate document called our Notice of Privacy Practices (HIPAA Notice). If the two documents ever disagree, the Notice of Privacy Practices controls for your health information.

What information do you collect?
Information you give us
When you fill out a form on our website, send us an email, or request an appointment, we collect what you give us. That is usually your name, phone number, email, and the reason you are reaching out.
If you apply for a job through our website, we also collect the information you submit: your work history, education, references, and anything else you include in your application.

Information we collect automatically
Like most websites, we log basic technical information when you visit. Specifically:
- Your IP address and general location (city level, from your IP)
- Your browser type and version, and your operating system
- The pages you view and the time you spend on them
- The website you came from, if any
- Timestamps of your visits
We use this information to keep the site running, fix problems, and understand what content is useful to visitors.
Cookies and similar technologies
We use a small number of cookies to make the site work and to understand general traffic patterns. We do not use advertising cookies or tracking pixels on pages that discuss specific conditions, symptoms, or providers. Do Not Track and Global Privacy Control
We honor Global Privacy Control (GPC) signals as a request to opt out of the sale or sharing of your personal information, where applicable. There is no single industry standard for Do Not Track (DNT) browser signals, so we do not respond to those signals directly. We limit tracking across the site as described in our Cookie Notice.
Social Security numbers
We do not collect Social Security numbers through our public website. If we need your SSN (for example, for insurance verification or billing), we will only collect it through a secure, non-public channel. We follow the North Carolina Social Security Number Privacy Act (N.C.G.S. § 75-62) and federal law for how we store, protect, and limit access to SSNs.
How do you use it?
We use the information we collect to:
- Respond when you contact us or request an appointment
- Run and improve the website
- Send the communications you asked for, like appointment confirmations
- Review and follow up on job applications
- Meet our legal obligations, including public health reporting
- Detect and prevent fraud or misuse of the site
Who do you share it with?
We do not sell your information.
We share limited information with:
- Other AMM Healthcare locations, so we can coordinate your care across the practice.
- Service providers, including our website host, our email service, our analytics provider, our secure intake vendor, and our appointment reminder vendor. Each of these is bound by a written agreement that requires them to protect your information.
- Authorities, when the law requires it (for example, a valid subpoena, public health reporting, or response to a credible threat to safety).
- A buyer or successor, if AMM Healthcare is sold, merged, or reorganized. We would give you notice if that happens.
What choices do I have?
Basic choices for everyone
You can:
- Turn off cookies in your browser settings
- Unsubscribe from our emails using the link in any email we send
- Ask us what information we have about you, and ask us to correct or delete it
- Tell us if you think we mishandled your information
Rights under state privacy laws
If you live in a state with a consumer privacy law (California, Virginia, Colorado, Connecticut, Texas, Utah, Oregon, Montana, Delaware, Iowa, New Hampshire, New Jersey, Tennessee, Minnesota, Maryland, Indiana, Nebraska, and others), you generally have the right to:
- Know what personal information we have about you.
- Access a copy of that information in a portable format.
- Correct inaccurate information.
- Delete information we have about you.
- Opt out of sale or sharing. We do not sell or share your personal information for cross-context behavioral advertising, but you can still make this request.
- Opt out of targeted advertising. We do not use your information for targeted advertising, but you can still make this request.
- Opt out of profiling that produces legal or similar effects about you.
- Not be discriminated against for using any of these rights.
To use your rights, email privacy@ammhealthcare.com or call (919) 535-7573. We will verify the request is coming from you before we act. We will respond within 45 days. If we need more time, we will let you know and may take up to 45 additional days.
Appeals
If we decide not to act on your request, you can appeal by replying to our decision email within 60 days. A different team member will review the appeal. We will respond within 45 days. If we still deny the appeal, you can contact your state Attorney General.
Authorized agents
You can ask an authorized agent to make a request for you. The agent must give us written permission signed by you. We may also ask you directly to confirm the request before we act.
Do you collect information from kids?
Our website is not meant for children. We do not knowingly collect personal information from children under 13, consistent with the federal Children’s Online Privacy Protection Act (COPPA).
In North Carolina, the age of majority is 18. If you are between 13 and 17, please use our website with a parent or guardian. A parent or guardian who believes a child has given us information through the site can contact us and we will delete it.
How do you protect my information?
We use commercially reasonable safeguards:
- Encrypted connections (HTTPS) across the website
- Access limited to employees who need it for their job
- Written agreements with vendors that require data protection
- Regular review of our security practices
No website is perfectly secure. Please do not send sensitive health information through email or web forms. Use our patient portal or call us.
How long do you keep my information?
We keep information only as long as we need it for the purposes in this policy, or as long as the law requires. Some examples:
- Contact form submissions: up to 3 years from your last contact with us
- Job applications: up to 2 years, unless you ask us to delete them sooner
- Website analytics data (not tied to you personally): up to 26 months
- Medical records: kept for the period required by North Carolina law and our clinical policies (generally at least 10 years for adults, longer for minors)
If you ask us to delete information about you and the law allows, we will do so.
What if there is a data breach?
If a security breach affects your personal information, we will notify you and, when required, the North Carolina Attorney General, the U.S. Department of Health and Human Services, and other regulators. We will send notice without unreasonable delay, consistent with the North Carolina Identity Theft Protection Act (N.C.G.S. § 75-65) and the federal HIPAA Breach Notification Rule.
What if this policy changes?
We may update this policy. If we do, we will update the date at the top of the page. For meaningful changes, we will also give you notice on the website and, where appropriate, by email.
